He was born on September 8, 1963 in Louisville, Kentucky to Richard and Helen Phillips. He is preceded in death by his Mother, Helen Diane Phillips.
Rick is survived by his, Father, Louie (Donna) Phillips, Sr., Sisters, Vickie Wheeler, and Carolyn (Keith) Hruska.
