Jack Quentin Keith, 45, of Louisville, Kentucky passed away on Tuesday, March 30, 2021.
He was born on January 27, 1976 in Louisville.
Jack is preceded in death by his stepmother, Lillie Bernice Keith; and his grandparents, Jack Stanley Keith and Anna Wilmajean Keith.
Survivors include his father, Jack T. Keith; brother, Jonathan Theodore Crawford (Keith) (Ashley); stepsisters, Tammy Crawford Bourgeois and Katie Crawford Nitting; stepbrother, Ricky Dale Crawford; and several aunts, uncles, nieces, nephews, and cousins.
A memorial visitation for family and friends will be on Saturday, April 10th, 2021 from 1:00 pm to 4:00 pm at Newcomer Cremations, Funerals, and Receptions, East Louisville Chapel.
