She was born on January 15, 1949 in Millerstown, Kentucky to Oree and Della Puckett. She is preceded in death by her parents.
Henrietta is survived by her Sister, Brenda Sue Puckett, Brothers, William Clark, Mark Clark, Randall Puckett, and Dennis Lee Puckett, Niece, Tina Beck, and Nephew, Michael Ray Puckett.
Visitation will be from 12:00 pm to 6:00 pm on Sunday, December 30, 2018 at Newcomer Cremations, Funerals & Receptions - Southwest Louisville Chapel, (10304 Dixie Hwy). A celebration of her life will be at 10:00 am on Monday, at the funeral home with burial to follow at New Lone Oak Cemetery, in Millerstown, Kentucky.
The family requests that contributions in Henrietta's memory be made to Alzhiemers research.
