Daniel, 55, of Louisville, passed away Tuesday, February 12, 2013.
He is survived by his children, Angela, Leah and Cody Hazelwood; parents, William and Mary Hazelwood and Doris Koenig; siblings, Brenda Mehler (Mike), Robbie Carter, Lisa Wethington (Bill), Keith Carter (Susan), Susan Hall, Tommy Koenig (Susan); three grandchildren and 10 nieces and nephews.
Funeral service will be held Sunday, February 17, 2013 at 5:00 pm at Newcomer Funeral Home, 235 Juneau Dr., Louisville. A visitation will be held Sunday prior to service from 1:00pm until time of service. Expressions of sympathy may be made to Kosair Children's Hospital.
